If you have not done so already, please review Step 1: Useful Information to understand the basics of the Mortgage Payment Assistance (MPA) Program. If you answered YES to all of those questions listed, please complete these steps below. If you did not answer YES to all of those questions, please call us for clarification.
Please print this packet and fill out the necessary pages. Along with these documents listed below, we will also need a copy of your most recent mortgage statement from your loan servicer (front and back pages).

Source of Income Documentation Form (1 page)
This is a list of documents you must bring with you. All income documentation must be collected for everyone age 15 and older. Everyone age 18 and older who receives income will need to sign this form at the appointment.
Income Certification (1 page)
This form must be completed in its entirety.
Duplication of Benefits (2 pages)
On page 2, leave item #4 blank. Our staff will fill that amount in later once we verify how much assistance the household will receive.
Intake Packet (14 pages)
This form must be completed in its entirety with all disclosures signed. If there is a significant other/spouse/other head of household living in the home, please include them as Co-Applicant, even if they are not on the mortgage loan or title.
Schedule Appointment
Once we have received all the documents and have reveiwed them, we will contact you to schedule a time to have a conference call with your mortgage servicer. This call will help us speed processes and gather information on your mortgage loan to discover what options might be available to get you current.
